Documentation is randomly missing from one or more libraries

I've got five projects as part of a documentation setup that have overlapping namespaces. The weird thing, is that each time I compile, the reflection engine works correctly, but it's randomly ignoring at least one of the .xml files that are generated. It's a different one each time.
 
I'm using SHFB.
 
I can't post a link or sample because of regards for my client's privacy, but I can provide other answers.

Are the assemblies and/or XML comments files using the same name? If so, that's the problem. SHFB copies the assemblies and comments files to a common location for the build and identically named items will overwrite each other and thus the information will not appear in the help file.

No. They're similar and there are references to each other though. They are:
(Client naming).Hardware.Common.dll
(Client naming).Hardware.SmartMotor.dll
(Client naming).Hardware.Laser.dll
(Client naming).Data.dll
(Client naming).Hardware.BarcodeScanner.dll

It's possible it's a SHFB issue. Can you send me an example project that duplicates the problem? My e-mail address is in the SHFB About box and in the footer of the pages of the SHFB help file.
