Missing Page in Website (404 error)

When creating a Website output in Sandcastle, if a user clicks to view the keyword index, a 404 page will be displayed in the main window.
 
This can be worked around by adding an index.aspx file in a new subdirectory called /undefined/ which redirects to another file of your choosing, however there should be something out of the box that at least prevents a 404 error from being in the output.

This is not part of Sandcastle. It's a feature of SHFB. Make sure you are using the latest release and, if it's still an issue, open a work item in the SHFB project (http://SHFB.CodePlex.com) and attach an example that demonstrates the problem.
